Satisfaction Survey for 2012
For your convenience, Cook's is providing TWO ways to submit your feedback.
If you wish you may print, fill out and drop off the attached survey at any time, or
you may use the attached online survey.
Please note that all responses received by Friday, February 24 will be tallied and
reported to the County of Northumberland Children's Services Manager as required annually.
We wish to encourage you to provide feedback by February 24, but we will gladly accept
any responses after that date. Your input is always welcome!
